Prior to your visit, it’s important to understand your insurance coverage. Different insurance policies offer different benefits. Per your insurance company, you may be partially responsible for the services we provide you:
A co-pay may be required, which is an amount collected when you check in for your appointment.
Co-insurance may be required, which is an amount remaining after insurance pays that is the patient’s responsibility. This would be billed to you after we have been notified by your insurance company.
If your deductible has not been met, this amount will be billed to you after we have been notified by your insurance company.
A referral from your Primary Care Physician (PCP) may be required. The primary care physician requests the referral from your insurance company. The referral is then issued to us. If we do not have a valid referral from your insurance at the time services are rendered, you may be responsible for the balance in full.
Some services may not be covered by your insurance policy. It’s best to call your insurance company prior to any service we provide so that you can be best informed about what you might be expected to pay.

If you do not have insurance, we offer a self-pay discount for our services. This policy does not apply to any expenses after your insurance company has made payment to us. Please call us at (425)673-5917 if you have questions about this discount.

If you cannot make your appointment time, you may be charged a fee for your missed appointment. We require a full business day notice for office appointment cancellations or reschedules. A fee of $30.00 is charged to your account for late cancellations or no shows to office appointments. For overnight Sleep Studies, we require two full business days notice of cancellation. Should you fail to notify us of cancellation of a Sleep Study within two business days, or do not show up for your Sleep Study, a fee of $75.00 will be charged to your account. The amount charged for a missed appointment is your responsibility and cannot be billed to insurance. We bill fees for missed appointments to defray the overhead cost associated with staffing and facilities for your visit.

After we have received a response from your insurance company for your visit, we will send you a statement asking for payment of the balance that is your responsibility. All charges are due by the due date listed on the statement. If you are unable to pay the balance in full, please contact our billing representative at (425)673-5917.
If you have insurance-related concerns, such as an issue with the amount of your deductible or how a service was covered, it’s best to first contact your insurance company directly. Your insurance company determines your financial responsibility for the services we provide, and has details about your policy that may not be available to our staff at Pacific Sleep Center. You can appeal any coverage decision made by your insurance company by calling the number on your insurance card or your Explanation of Benefits.
If you think a charge on your statement is incorrect, please notify us at (425)673-5917 within 14 days of the receipt of your first statement.

To pay your bill, we accept cash, checks, money orders, or Visa/Mastercard credit or debit. You may pay in one of the following ways:
Pay in person ~ Pay your bill in person at our Edmonds office during business hours: Monday- Thursday 8am-5pm and Friday 9am-12pm
Pay by mail ~ Submit payment via mail by sending it to Pacific Sleep Center 22005 76th Ave W Edmonds WA 98026
(If you do not have the remittance slip from your statement, please be sure the patient’s name is listed on the check or a separate sheet of paper.)
