Feel free to bring your sleep partner with you for your visit. He/she may know more details of your sleep activity (how often you snore, gasp or have limb jerks during sleep).

Medical History Questionnaire (click here to print form online - PDF format) Take note of your sleep schedule and observe certain habits. These observations will be helpful when filling out our History Questionnaire. The questionnaire is about 4 pages long so be sure to allow plenty of time to fill it out prior to your appointment. The information you provide in the questionnaire will be extremely useful to Dr. Johnson in making decisions for a course of treatment.

Co-Payment If your insurance policy requires that you pay a co-pay, we are REQUIRED by your insurance company to collect this amount from you at the time of service. You may pay by cash, check, Visa/Mastercard credit or debit card. If co-payments are not received at the time of service, a $3.00 billing fee will be charged to your account.

What to Expect During and After Your Visit
A new patient evaluation typically lasts about a half hour. During that time Dr. Johnson will assess your concerns, review the medical history questionnaire with you, perform a focused physical exam, and answer any questions you might have.
Dr. Johnson may recommend that you undergo an overnight sleep study at our clinic, which will be scheduled after your first visit. The sleep study allows us to determine the issues that are at the core of your sleep problems. A follow-up office visit is also scheduled approximately two weeks after your sleep study, and during this time Dr. Johnson will review the results of the sleep study with you and recommend treatment options.
